Since my older SB6141 was having problems, i went ahead purchased a Netgear CM500-100. Couldn't activate it online and had to call. Unfortunately the call lasted for more than 30mins. as the rep was having a hard time, even she admitted. Finally got it activated and provisioning seems to be correct. Getting 16 down & 4 up bonded channels and speeds are acceptable with this new modem on my plan. 198dn/22up
6141's will only be provisioned up to 100/10 service.
There's a second provisioning group that's good if you can get connected to them.
Also copy and ppaste the modems signal level page and your zip code, let's make sure there's not a level problem...

Levels look good, print out a copy for your records should connections deteriorate.
