I received a private response from @agentx5 that he could not confirm a code update was needed by or had been pushed to this subscriber's specific COAM internet cable modem unit. To clarify, not all SB6190s need the fix; later production runs of the SB6190 were built and shipped with a Puma7 Intel processor.
Contact Arris for a warranty replacement due to improper design? They were replacing 6183's in early 2016/late 2015 due to FW issues
In the modems "about" or "Help" screens, it should have a FW date... If it's still early 2016, it was not updated.

this looks like a bad amplifier somewhere.
It could be a bad modem but surely the tech would of blamed it if it's coam or replaced it if a leased one...
Oh wait, you mentioned "SIGNAL BOOSTER" what is it's exact model number and how is it connected? A photo may help as well. We caught TWC improperly installing a lot of a certain model...
Well, today things look different. I'm wondering if TWC have ahd some other problems outside our house that they may have now fixed (well, hoping)!

my opinion is no, not fixed.. What is the exact model number of the amplifier and where is it connected?
The amplifier is at the entry box into the house. It has no label or distinguishing features. The power supply is inside the house and powers the amplifier through the coax.
ok, is there a seperate coax between the amp and the wall wort? Or is it shared with a cable device and has a Diplexer (looks like a splitter) to add power to the coax and seperate the cable rf It will have 3 ports, one marked amp, the other marked power.
What's marked on the power supply wall wort? (yeah I know it's 12vdc...)
Does the amplifier have multiple splitter ports on it or is there a seperate splitter?
Is it silver plated or grey painted, what color is the power LED?
TWC installed many of this manufacturers wrong... especially the ones without the RED (power only) port....
