Connections are somewhat backwards, should be split to the modem first, that will lower US by 3.5 dB and raise the DS by 3.5 dB
That's the first thing I'd change to see if it increases the reliability, but there's probably another issue, maybe outside

Print that out for the tech as well.. real bad, one upstream channel maxxed out at +53 dBmv and only one downstream channel.
You have some sort of coaxial issue for a start.
Just changed out modem and wiring to the modem. Instead of the UBEE I have a Technicolor one now. All the connectors inside and out were also changed.
