They have a special head-end which serves the building.   This not only decrypts the signal from the street, but also amplifies it for distribution through the many rooms of the hospital.  Hotels work the same way.

 

It's not available to the public for residential use.

A guy I know saw a new Spectrum Guide at the hotel where he was staying in New York City.  It was a blue colored HD guide that I had not seen on any home systems.  It didn't really look like the Cloud-Based Spectrum guide that is coming out either.
