Intermittent internet outages

I've been having this problem for a few months, but the frequency has been increasing.  The internet connection goes out anywhere from a couple of minutes to as much as 20 minutes.  I've been checking the modem logs and I have a lot of T3 and T4 errors with modem reboots.  I tried the modem with and without the power booster upstream  but it doesn't seem to have any effect.  I checked the cable and all the connections.  Any help would be a appreciated.  I'll paste the modem logs below:

While not as detailed as the info you provided, I have noticed my internet has been going out around the same time for the past week (between 7-8pm CST).  Possible throttling from the service provider? :/

Nope...this is a signal issue. Somewhere you have loose connections, corroded connectors, chewed up cable, unterminated jacks, etc. that are causing the problems. @sjstempa Please also post the signal levels page from the modem.
